Alex Pereira began his journey in combat sports in 2009 training as a kickboxer in order to get rid of his alcohol addiction. Three years later, he was crowned the Brazilian national champion.
Pereira would then start fighting for Glory Kickboxing, winning the Glory 14: Zagreb – Middleweight Contender Tournament in Zagreb in March 2014. During his Glory days, he won the middleweight and light-heavyweight crowns. He also recorded victories against notable fighters such as Jason Wilnis, Maycon Silva, Artem Vakhitov and Israel Adesanya.
Pereira started transitioning to MMA in 2015 and lost his first fight via submission. The following year, he won his two fights via knockouts. In October 2020, he signed with Legacy Fighting Alliance and won his first fight via knockout in the first round.
On 3 September 2021, Pereira was signed by the UFC. He fought three times in the promotion before challenging Adesanya for the Middleweight Championship at UFC 281. He won the fight but his reign wa short-lived as Adesanya regained the title at UFC 287.

Pereira then moved to the light heavyweight division where he defeated Jan Blachowicz on his first fight. On 11 November 2023, he defeated Jiri Prochazka and became the UFC Light Heavyweight Champion. Following his victory, Pereira became the ninth fighter in the UFC to become champion in two weight divisions.
